What Is Medicare Supplement Insurance (Medigap)?
Medicare Supplement Insurance — also called Medigap — is a private insurance policy that helps pay costs Original Medicare doesn't fully cover. This includes things like copayments, coinsurance, and deductibles.
You keep Original Medicare as your main coverage. Medigap works alongside it to cover your share of the bill — making your healthcare costs more predictable.
To buy a Medigap policy, you must be enrolled in Medicare Part A and Part B. Plans are sold by private insurance companies but are standardized by federal law. That means Plan G from one company covers the exact same benefits as Plan G from another — the difference is only the monthly premium and the carrier.

Note: Medigap is not the same as Medicare Advantage. You cannot have both. Medigap also does not include prescription drug coverage — you would need a separate Part D plan for that.
Which Medigap Plan Is Right for You?
There are ten standardized Medigap plan letters. Here are the four most commonly chosen by Medicare beneficiaries:
Plan G — Most Complete Coverage for New Enrollees
Plan G is the most comprehensive plan available if you became eligible for Medicare on or after January 1, 2020.
What Plan G covers:
- Part A hospital costs and coinsurance
- Part A deductible ($1,736 per benefit period in 2026)
- Part B coinsurance — the 20% you would owe on most outpatient visits
- Part B excess charges
- Skilled Nursing Facility coinsurance (days 21–100)
- Hospice care coinsurance
- Foreign travel emergency care
The only cost Plan G does not cover is the Part B annual deductible ($283 in 2026). After you pay that once per year, Plan G covers the rest.
Plan N — Good Coverage at a Lower Premium
Plan N offers similar protection to Plan G, but typically costs less per month. The trade-off is small copayments when you visit the doctor or emergency room.
What Plan N covers:
- Part A hospital costs, coinsurance, and deductible
- Part B coinsurance (except a copay of up to $20 for some office visits and up to $50 for ER visits that don't result in hospital admission)
- Skilled Nursing Facility coinsurance
- Hospice care coinsurance
- Foreign travel emergency care
Plan N does not cover the Part B annual deductible or Part B excess charges. It is a popular choice for people in good health who want solid coverage with lower monthly premiums.
Plans K and L — Lower Premiums with Cost-Sharing
Plans K and L have lower monthly premiums. In return, you share more of the costs — but each plan has a yearly out-of-pocket maximum that caps your exposure.
- Plan K: covers 50% of most Medicare cost-sharing. Out-of-pocket cap: $8,000 per year in 2026.
- Plan L: covers 75% of most Medicare cost-sharing. Out-of-pocket cap: $4,000 per year in 2026.
Once you reach your plan's annual limit, it pays 100% of covered costs for the rest of that year. These plans work well for people who want a lower premium and are comfortable with a defined cost-sharing structure.
What Medigap Does Not Cover
No Medigap plan covers the following:
- Prescription drugs — you need a separate Part D plan
- Routine dental, vision, or hearing care
- Hearing aids or eyeglasses
- Long-term care or custodial nursing home care
- Private-duty nursing
Each Medigap policy covers one person only. Couples must each purchase their own separate policy.

When Should You Enroll?
Timing matters more with Medigap than with almost any other insurance decision.
Your best window is the Medigap Open Enrollment Period — the 6 months starting the first month you have Medicare Part B and are age 65 or older. During this window, insurance companies cannot deny you, cannot charge you more due to health conditions, and cannot make you wait.
After that window closes, most states allow medical underwriting. That means insurers can review your health history and may charge more — or decline to cover you altogether.
There are exceptions called guaranteed issue rights, which may apply if you lose other coverage or return from a Medicare Advantage plan. A licensed advisor can help you understand your rights before your window closes.
